Vaccination Information
The Family Health Centers, Inc. (FHC) will host COVID-19 walk-in vaccination sites to provide second doses of the Moderna vaccine to previously vaccinated individuals, as well as first doses to individuals age 18 and older. If you are receiving your second dose, please bring your COVID-19 vaccination record card and photo ID. FHC is now providing second doses of the Moderna vaccine to individuals who received their first dose at a different location. Vaccinations will be administered while supplies last. Monday through Friday from 8:30 a.m. to 4:00 p.m.
